          For the Commonwealth of Massachusetts and the Board of Registration in Dentistry: Elizabeth Kaplan, BBO#568911, Assistant Attorney General, Boston, MA.

          For Donald Sydor: Philip D. Moran, BBO#353920, Salem, MA.

         Whereas, Donald Sydor acknowledges that he engaged in the unlicensed practice of dentistry in violation of statutes, orders of the Board of Registration in Dentistry, and the Court's April 6, 2015, order;

         Whereas, Donald Sydor further acknowledges that this Court has subject matter jurisdiction over this case and personal jurisdiction over him, and consents to the entry of this Final Judgment in the above-captioned case; and

         Whereas, Donald Sydor waives all rights of appeal and also waives the requirements of Rule 52 of the Massachusetts Rules of Civil Procedure.

         IT IS HEREBY ORDERED, ADJUDGED AND DECREED that:

         1. This Court has jurisdiction over the subject matter and the parties to this action, which include the Commonwealth of Massachusetts, the Board of Registration in Dentistry, and defendant Donald Sydor. Venue in this Court is proper under G. L. c. 93A, § 4, and G. L. c. 223, § 5. The Attorney General is authorized to bring this action under G. L. c. 112, § § 43 and 65, and G. L. c. 93A, § 4.
